Delivers coordinated nursing care for Acute Coronary Syndrome (ACS) and stroke patients to established standards of care and the nursing process. Supervises and directs the activities of various levels of assigned nursing staff, and coordinates care with other disciplines while utilizing critical thinking, professional and supervisory discretion, and independent judgment.

Performs rounds with the Emergency Department, Cardiac Cath Lab, Cardiac Telemetry Unit, and other Inpatient Care Units as required. Conducts ongoing review and education on Acute Coronary Syndrome disorders to include all shifts, ensuring that the elements of Acute Coronary Syndrome and the cardiac program are in place and functioning appropriately.Acts as clinical resource to staff by providing in-services, validating cardiac-related competencies, and advising management of training needs. Develops and implements clinical pathways and gold standards, internal and external clinical education programs in conjunction with the Society of Chest Pain Center, American College of Cardiology, and the American Heart Association. May provide education and collaboration for pre-hospital care organizations.Coordinates all of the disciplines involved in the care of the patient to ensure appropriate cost effective and efficient progression through the system and continuity of care. Leads multidisciplinary team efforts to identify clinical problems and needs. Designs processes to implement and manage patient transitions through the system pre- and post-discharge evaluates care to assure desired patient outcomes.Compiles and submits all necessary information and data to the appropriate agencies in preparation for the accreditation/certification inspection. Serves as the point of contact for the program to both internal and external customers and healthcare providers. Maintains and ensures continued success of the program by complying with the requirements for ongoing data submission and outcomes monitoring as outlined by the appropriate agencies.Performs other job-related duties as assigned.
