Adventist Health Clear Lake is a 25-bed, critical access hospital serving Lake County with key service areas including an emergency care, imaging, intensive care, maternity, physical rehabilitation, pulmonary rehabilitation, surgery and nine rural health clinics that provide behavioral health, cancer, dental, family medicine, OB/GYN, pediatrics, neurology, orthopedic and pain management to name a few.

 

We're looking for someone to join our team as a RN Cardiovascular Clinical Analyst in our new Cardiac Rehab Unit.  This Analyst will support the cardiovascular program coordinators and be responsible for data management and assists with coordinating and facilitating care for cardiovascular and stroke patients throughout a continuum. Collaborates closely with physicians and nurses to ensure clinical care paths are followed, as appropriate. Assists stroke and chest pain coordinators with a variety of tasks for cardiovascular and stroke programs, including data gathering and input, staff and patient education, community education and marketing.

 

Essential Functions:

  • Assists stroke and chest pain coordinators and service line directors with preparing for cardiovascular and stroke program promotion and marketing. 
  • Assists stroke and chest pain coordinators with code stroke responses and code ST Elevation Myocardial Infarctions (STEMI). 
  • Helps stroke and chest pain coordinators prepare for and complete staff education.
  •  Assists stroke and chest pain coordinators with The Joint Commission (TJC) certification preparation and Society of Cardiovascular Patient Care (SCPC) certifications. 
  • Assists stroke and chest pain coordinators with in-patient and family education, as needed. 
  • Helps stroke and chest pain coordinators plan community education events for healthcare professionals and lay communities. 
  • Assists stroke and chest pain coordinators with daily operations and cardiovascular certification program maintenance.
  • Assists stroke and chest pain coordinators with discharge phone calls. 
  • Attends and participates in required cardiovascular and stroke center meetings, as appropriate.
  • Completes data extraction under the supervision of cardiovascular program coordinators for cardiovascular and stroke program measurements. 
  • Enters program outcome information into designated databases. 
  • Completes data spreadsheets for additional performance improvement and quality assurance initiatives. 
  • Develops data sheets for ongoing program changes. 
  • Obtains charts for data extraction and prepares daily CV and stroke program census. 
  • Resolves system related problems pertaining to database operations.
  • Coordinates with other members of the interdisciplinary health care team to develop clinical care pathways for cardiovascular patients and to achieve desired goals and outcomes. 
  • Provides clinical resources and consultation to staff about cardiovascular patients with clinical complex needs.
  • Performs other job-related duties as assigned.

You'll be successful with the following qualifications:

 

Education: 

  • Bachelor's Degree (BSN): Preferred
Work Experience:

  • Three years nursing experience with experience caring for cardiovascular patients (Cath Lab, Non-Invasive Cardiology): Preferred
  • Experience in data collection, medical records or related field: Preferred
  • Experience with stroke patients: Preferred

 

Required Licenses/Certifications: 

  • Registered Nurse (RN) licensure in the state of practice: Required
