We're looking for someone to join our team as a Program Specialist, Wound Care who:

Leads the organization's efforts to ensure quality outcomes are achieved for the wound care patient throughout the patient care experience. Manages program with complete discretion and independent judgment. Supervises and directs the activities of various levels of assigned personnel using both professional and supervisory discretion and independent judgment. Serves as a consultant in the management of patients with draining wounds, fistula and tubes and for the treatment of patients with altered skin integrity due to pressure necrosis, drainage or incontinence.

 

Essential Functions:

  • Works with physicians and nursing staff to provide updated information regarding wound care. Conducts rounds on all patients with wound/pressure ulcers on a daily and as needed basis. Provides care to medically complex patients in collaboration with the physician in a specialist acute care setting. Works autonomously and has authority, responsibility and accountability for assessment, planning, implementation and evaluation for wound, ostomy and continence care program throughout the inpatient areas. Serves as a resource for wound care management, including identification, assessment, treatment and prevention.
  • Provides specialized consultation on wound, ostomy and continence issues for patients, families, nursing and medical staff. Coordinates all disciplines involved in the care of the patient to ensure appropriate cost effective/efficient progression through the system and continuity of care. Leads multidisciplinary team efforts to identify clinical problems and needs. Evaluates care to ensure desired patient outcomes. Analyzes patient data collected, outcome monitoring and trend reporting surrounding condition management issues.
  • Performs rounds with appropriate staff/departments, as required. Conducts ongoing review and education to all shifts, ensuring all elements of the wound care program are in place and functioning appropriately. Consults with physicians and other healthcare professionals, as indicated, regarding patient care and quality of care issues for the population served. Collaborates with the team of patient, family and healthcare providers in providing patient care in a safe, healing, humane and caring environment.
  • Provides learning opportunities for patients/family members and team members. Assesses educational needs of hospital staff related to wound care. Plans and participates in educational activities for patients, families, staff, physicians and community. Provides health information to patients, families and treatment team. Collects relevant data pertinent to the patient's health or situation. Analyzes assessment data in determining diagnosis and care issues.
  • Develops a plan that prescribes interventions to attain outcomes. Implements the plan, coordinates care delivery and employs strategies to promote health and a safe environment. Evaluates progress toward attaining outcomes. Identifies outcomes for the patient's situation. Maintains appropriate supply of equipment and materials. Maintains complete, accurate, concise, legible documentation of patient status, interventions and responses.
  • Performs other job-related duties as assigned.

 

You'll be successful with the following qualifications:

 

Education: 

  • Bachelor's Degree in Nursing (BSN): Preferred

 

Required Licenses/Certifications: 

  • Registered Nurse (RN) licensure in the state of practice: Required
  • Case management certification: Preferred
