Adventist Health Home Care Services brings quality care to home bound patients in the comfort of their own home throughout the greater Portland area. At Adventist Health, Home Health is available around the clock, including weekends and holidays. We know that patients facing a serious illness, or recuperating after a surgery generally prefer the comforts of home for their recovery. Our staff creates an individual plan of care for each patient, based on the patient's physician's orders to help manage care effectively with the utmost respect for the patient's dignity, privacy and well-being.

We're looking for someone to join our team as a LPN (Hospice) who:

Performs procedures which require specialized training for general nursing care and related services for assigned patients, under the direction of a Registered Nurse (RN) or Medical Doctor (MD), in accordance with established policy. Participates in the planning for and provision of patient care. Uses knowledge of asepsis in performance of treatments, such as sterile dressings, irritations and cauterizations. Provides functional direction to ancillary personnel.

Schedule: 8 hour shifts (8am - 5pm with variable shifts), Monday - Friday with weekend rotation

Essential Functions:

  • Provides care for assigned patients, under the supervision of an Registered Nurse (RN) according to established standard and nursing process.
  • Performs admission assessments, and ongoing re-assessments based on departmental specific policy and procedures. 
  • Assists in the development of a prioritized interdisciplinary plan of care on each patient.
  • Implements and coordinates plan of care utilizing appropriate organizational skills and reporting mechanisms. 
  • Implements individual teaching plans, including Patient Teaching Protocols and discharge Plans, based on mutual goal setting 
  • Performs assigned clinical procedures (i.e. phlebotomy, injections, drug screens, etc.) 
  • Administers medications.
  • Orders/restocks supplies as assigned, according to established par levels. 
  • Maintains neatness/cleanliness in assigned areas.
  • Performs other job-related duties as assigned.


You'll be successful with the following qualifications:



  • Licensed Practical Nurse diploma/certificate: Required

Work Experience:

  • At least one (1) year nursing experience in hospice, acute care or skilled nursing: Preferred

Required Licenses/Certifications: 

  • Basic Life Support (BLS) Health Care Provider certification: Required
  • Valid Driver's License (DL): Required
  • Current Licensed Practical Nurse (LPN) license for State of Oregon: Required
